Mohan M Chilukuri, MD
Dr. Chilukuri is Board Certified in Family Practice and Geriatrics. Dr. Chilukuri graduated from medical school in 1982 at the University of Kentucky and completed his residency at Trover Clinic Foundation/Regional Medical
Center, Madisonville, KY.
Dr. Chilukuri's areas of Medical Interest include: Preventative Medicine, Children's Health, General Family Medicine.
Rashida Coleman, FNP-BC
Ms. Coleman is a Dual-Board Certified Family Nurse Practitioner. She received her Bachelor of Science in Nursing from Winston Salem State University in 2018 and her Master of Science with a concentration of Family Nurse Practitioner from UNC Wilmington in 2021.
Ms. Coleman has a passion for Women’s Health and Preventative Medicine
Caitlyn Slonaker, FNP
Caitlyn Slonaker is a Certified Family Nurse Practitioner. She received her Masters of Science in Nursing with a concentration in Family Practitioner from Virginia Commonwealth University. She is an active member of the American Association of Nurse Practitioners, NC Nurses Association and American Nurse Association. Mrs. Slonaker has a passion for Women's health, Mental Health and Preventative Medicine
John "Ben" Johnson, FNP
John Johnson, better knows as"Ben" is Board-Certified by the American Academy of Nurse Practitioners as a Family Nurse Practitioner. He received his Bachelors of Science in Nursing from NC Central University in 2016 and his Masters of Science in Nursing from Simmon's University in 2022. His Medical interest include Preventative Medicine, Chronic Disease Management, Integrative and Functional Medicine.
